A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $3,000 was issued for a Severity Level III violation involving the failure to secure from unauthorized removal or limit access to two gauges containing approximately eight millicuries (mCi) of cesium-137 and 50 mCi of americium-241 and failure to control and maintain constant surveillance of this licensed material.
